 Prior Approval Requirements

  1. Prior approval for use shall be obtained whenever any of the following apply:

    1. Quantities of chemical(s) to be used are greater than that which would be considered laboratory scale (see Definitions section).

    2. The material is Extremely Hazardous, a reproductive toxin or a select carcinogen, which has never been used in that specific laboratory before.

    3. Lack of or inadequate ventilation exists.

    4. A Material Safety Data Sheet (MSDS) is not available.

  2. Persons who may grant such an approval are the Department Chairperson and/or the Laboratory Chemical Hygiene Officer (LCHO) with appropriate notification to the Campus Safety Department.

  3. A record of the approval should be entered into the lab notebook for the experiment.
