9.00 Licenses and Registrations

Union College has been issued a specific license by the New York State, Department of Health, to cover the uses of radioactive materials on the Union College campus and satellite facilities.  Copies of this license are available in the Office of the Radiation Safety Officer.

Union College has also been issued a Certificate of Registration for machines that produce ionizing radiation.  This registration is available in the office of the Radiation Safety Officer.

Each individual wishing to use radioactive material and/or radiation generating devices must make proper applications to the Radiation Safety Officer.  Form 9.1 should be used for these applications.  The applications and registration requests  must be signed by the Department Chairman, Center Director, or appropriate Dean.  The RSO will provide assistance on all applications.  The "owner" of the facility will serve as Radiation Installation Safety Officer.  Copies of all applications, licenses (when issued), registrations (when issued), feasibility reports, hazards analysis, etc., must be transmitted to the Radiation Safety Officer at the time they are submitted to or received from any outside agency  The application must include a completed copy of Form 15.1, "Statement of Training and Experiences for Authorized Users."  A copy of the form is found in Section 15.00, Radiation Protection Training.
