STEP 1: FORM 1 - Application for Licensure and First Registration

All applicants for licensure must complete this form and submit it with the appropriate fee ($70) directly to the Office of the Professions at the mailing address at the end of Form 1.  Make checks payable to the New York State Education Department.  Your cancelled check is your receipt.  You must answer all questions and provide all information requested unless otherwise indicated.  Failure to complete all required parts of the application will delay its review.  Your signature on Form 1 must be notarized by a notary public.  Upon receipt of your completed applicaton, we will send you an acknowledgement letter.  Form 1 and the $70 fee are due in the Office of the Professions by 1 November 2008.

STEP 2: FORM 20 -  Certification of Enrollment in a Professionally Registered Program in Engineering

 For use by New York State Institutions with EAC/ABET accredited programs in Engineering only.  Not to be used by individuals.  This form must be submitted directly by the educatonal institution.  The Office of the Professions will not accept this form if submitted by the applicant. 

(The Union College Registrar will be submitting this form directly to the Office of Professions.  This form is used by academic institutions instead for Form 2.  You do NOT have to fill out Form 2.  Contact Rhonda Becker in the ME office to make sure that you are being included on this form and that the information is correst.  Her extension is x6264)

 STEP 3: REGISTER and PAY EXAM VENDER

This FE exam is a national exam that is administered by an exam vender, not New York State.  The exam vender will send you an exam scheduling information in December, it will be mailed to the address that you used on Form 1.  You will need to provide the exam vendor with some additional information and send in another check for the exam fee (typically $135).  If you miss the due date you will not be allowed to take the exam.  Once the exam vendor receives you information and fee you will receive a registratoin card that admits you to the exam.
