Financial Aid Forms

Financial aid forms are year-specific. Please make sure you scroll down and select the form for the correct year for which you are applying for financial aid. All forms must be complete and submitted to the financial aid office by the indicated due date in order to be considered for financial aid. For more information on due dates, please review the Application Process for Incoming Students or Returning Students.

Dependent Verification Worksheet
To be completed by dependent students and their parent(s).

Independent Verification Worksheet
To be completed by students who meet the 'independent student' status as determined by the FAFSA.

Returning Student Employment Application
To be completed by returning upperclass students who would like to continue to participate in the work study program.

Incoming Student Employment Application 
To be completed by freshmen or transfer students who have been awarded work study as part of their 2014-2015 financial aid package.

International Student Financial Aid Application                   
To be completed by International Students from Benin, Cameroon, Cuba, Ghana, Iran, Nigeria, Sudan and Togo who are not able to complete the CSS Profile. This application must be completed to be considered for Financial Aid. The due date for prospective students is February 1, 2014.

International Certification of Finances                      
To be completed by first-year, international students who have been admitted and are enrolling at Union.  The due date for this form is May 1, 2014.

Change in Family Circumstances Form                   
Families who have experienced a change in circumstances after filing financial aid application materials should complete this form and submit it to the financial aid office with any requested supporting documentation.

Parent (PLUS) Loan Information Sheet                                                    
To be completed by parents who wish to borrow a Direct Parent (PLUS) Loan. The form also includes instructions for completing a credit check and Master Promissory Note.
