(Bio 080) Guidelines

You will receive an information packet from Cathie Pastore at Ellis Hospital that will include all necessary information and paperwork for you to complete in order to intern at Ellis.

The health clearance form must be completed before you can begin your internship or observation hours. Due to HIPAA regulations, you will need to sign a release of information. Health Clearance Form Release Form

A physical exam is required if one has not been done within 12 months from when you will be completing the internship. Please arrange for the physical with your private physician.

A PPD (tuberculosis) test within the last twelve months is required. If your private physician or the Union health office cannot perform the test, you may have it performed at the Occupational Health Office at Ellis free of charge any weekday but Thursday
7 am-4 pm. You will have to complete the two-step PPD screening. See below.
If you have not had any PPD within the past 12 months, then you must have the two-step PPD which involves having a second PPD shot 2-3 weeks after your first one is read as negative. Therefore, the PPD clearance will take you about three weeks.

Your immunizations for measles, mumps, and rubella must be up to date.
Current immunization for tetanus/diphtheria within 10 years is recommended.
Positive chickenpox history/immunization is recommended.
Hepatitis B vaccination or informed refusal is recommended for students in hands-on care areas.
A flu shot is also recommended but not required.
All other required paperwork and forms must be returned to Cathie Pastore before beginning the experience.
