It is both a New York State Law and the policy of Union College that fully completed Health and Immunization Records must be on file prior to class attendance.
Therefore, all forms must be completed and returned to Health Services by June 15.
EXPLANATION OF PORTAL HEALTH FORMS
1. Complete Insurance Information Form, as it is mandatory to supply the UC Health Center with your insurance information.
FYI:There is no charge for your child to visit the UC Health Center. Limited quantities of basic over-the-counter medications and most in-house laboratory testing are provided at a minimal cost. Fees are charged for immunizations, prescriptions medications dispensed from our pharmacy, and certain other laboratory services provided by the UC Health Center. The UC Health Center does not bill either students’ private insurance or Student Health Insurance for services rendered. Students (or their health insurers) are responsible for all bills relating to outside laboratory tests and x-rays, prescription medications and immunizations, emergency room and/or urgent care center visits, and visits to medical specialists.
Adequate health insurance coverage is essential to avoid out-of-pocket expenses and/or delays in accessing services in the event off-campus medical care becomes necessary. As many private Health Insurance Plans, especially HMO’s, may not provide coverage for non-emergency, out-of-network medical care (including laboratory services, x-rays, specialist referrals, etc.), or may require prior authorization from your primary health care provider. Union College offers a reasonably priced, primary student insurance plan with Gallagher Koster. The Finance Office will be providing information about the Health Insurance Plan available to Union College students. You will be asked to either select enrollment in the Student Health Plan or provide proof of your alternative coverage. Although your insurance information is requested by the UC Health Center, you must also provide information on the selection or waiver to be returned to the Finance Office. Students for whom College Finance does not receive proof of alternative coverage will be automatically enrolled in the Student Health Plan, and the cost will be charged to their student account. Be sure that you, together with a parent/guardian complete the Personal Health History to ensure accurate medical history information.
2. ONLY if you are under 18 years of age: Your parent/guardian MUST print, sign, date, have notarized, and return the Medical Treatment of Acknowledgement Consent Form and the Release of Medical Information Consent Form to: Union College Health Center, 807 Union Street, Silliman Hall, 2nd Floor, Schenectady, NY 12308 by June 15.
3. A physical examination is required within 12 months prior to your arrival on the Union College campus. The Physical Examination Form, Immunization Verification and Tuberculosis Risk Form are to be printed, completed and signed by a Health Care Provider and returned to: Union College Health Center, 807 Union Street, Silliman Hall, 2nd Floor, Schenectady, NY 12308 by June 15.
REMEMBER:
*New York State Law is very specific about the measles, mumps and rubella immunization requirements, an MMR vaccine will have to be repeated if given within the first 12 months of birth per New York State Law.
*Union College requires date of a Tetanus-Diphtheria-Pertussis booster within the past 10 years.
*Union College follows the CDC New Meningococcal (Meningitis) vaccine requirement of:
Vaccine #1 age 13-15 and Vaccine #2 age 16-18 OR 1st dose age 11-12, booster at age 16 OR Single dose over age 16.
*In addition to your immunizations, a Tuberculosis Risk Form MUST be completed. If you need a PPD (Tuberculosis Test) it must be placed and read within the past year to be valid. Or, if you have a history of positive PPD test, a current chest x-ray must have been done within the past year.
