Confidentiality
All records are confidential. No information will be released without the student's written permission. The one exception to this is in a life-threatening situation when it may be necessary to release information on a need-to-know basis. You may obtain a Release Of Information form from the UC Health Center office.
Class Excuses
The UC Health Center will not write notes excusing students from their academic and/or extracurricular responsibilities. If a student does not feel well, he/she should visit the UC Health Center so an assessment can be made and a record of the illness documented. If a professor calls the UC Health Center, we will confirm if a student was ill without releasing any specific information. Students should communicate directly with professors concerning class absences and missed assignments. After a student has missed three days due to illness or injury, we will notify the Dean of Students Office, which in turn notifies professors of a student's absence.
Transportation
Any ill or injured student requiring transportation to the UC Health Center or a local hospital will be transported by Campus Safety. Any student needing emergency transportation should contact Campus Safety first, and they will contact the local ambulance service and paramedics. Only approved personnel may transport ill or injured students due to liability issues.
Transportation to medical appointments within a six-block radius of campus can also be provided by Campus Safety.
A taxi service with special rates can be utilized for medical appointments outside the six-block radius of campus by calling Health Services.
Expenses
There is no charge to see a health care professional at the UC Health Center. However, students will be charged for supplies and medications. These items may be paid by check or cash or placed on the student's bill. Medication costs are the responsibility of the student.
Students who choose to add in-office medical services to their tuition bill will have a Health Services fee shown on the bill. The UC Health Center will not release information regarding this fee to parents unless the student gives written permission to do so.
Our dispensing unit is not a licensed pharmacy. We supply pre-filled generic medications at a reasonable price through a contract with a pharmaceutical company. Some insurance plans do not cover the cost of these generic medications. Parents, please review your insurance policy and advise your child accordingly. We are unable to honor insurance prescription cards at the UC Health Center. Students may request a written prescription and have it filled at a local pharmacy. Lange's Pharmacy delivers once a day to our office. You may want to contact them at (518) 374-3324 for information regarding your insurance plan.
Blood work and laboratory specimens are done at the UC Health Center and sent to Ellis Hospital through a courier service. X-rays are done at Ellis Hospital. The hospital bills directly to the student's home address. All Ellis Hospital costs for x-rays, laboratory procedures, emergency services, etc. are the responsibility of the students.
Due to the high demand for appointments with Diane Pietrocarlo, RD and the high number of students “no showing” their appointments with her, the Health Center has been forced to implement a “No Show Fee”.
If a student “No Shows” their appointment with Diane Pietrocarlo, RD and has not called to cancel their appointment 24 hours before their scheduled appointment, their student account will be charged: $30 for Intake (New Visit) Appointment / Follow-up Appointment

SENIOR BILLING CUT-OFF IS May 1st. Seniors may not bill tuition for medications, tests, supplies through the UC Health Center after May 1st. Payments must be by cash or check. Credit cards are welcome for Lange's Pharmacy prescriptions.
Insurance
We invite you to learn more about our student health insurance by visiting Gallagher Koster. You do not need to have the school health insurance to be seen at Health Services by a nurse or provider.
