Medical Insurance Options

You have the opportunity to select one of three Medical Plans which best meet your health care needs. The following provides a summary of those options and a Schedule of Benefits for each plan is enclosed. Complete details of each option are also contained in the Summary Plan Description available from the Human Resources Office. You may also choose to opt out of medical coverage if you and your family can show proof of coverage elsewhere.

The UC POS plan is a Point Of Service plan administered by CDPHP. To enroll in the UC POS PLAN you must select a Participating Primary Care Physician at time of application. You have a great degree of flexibility with the UC POS PLAN. You may choose to use participating physicians and hospitals. This is called "IN-NETWORK". You may choose to use non-participating physicians and hospitals. This is called "OUT-OF-NETWORK". You can use a combination of "IN-NETWORK" or "OUT-OF-NETWORK" providers. Your out-of-pocket cost will vary depending upon how you use the UC POS PLAN.

The "IN-NETWORK" benefit is designed to save you money and is easier to use because there are no forms to fill out. Choosing the "OUT-OF-NETWORK" benefit is entirely up to you and is totally permissible. To use the "IN-NETWORK" system you select a Primary Care Physician from among private practice participating doctors. The Primary Care Physician is chosen when applying for coverage and can be changed at any time. If you have a family, each family member may have their own Primary Care Physician. General health care, referral to other doctors, and hospital admission must be directed by the Primary Care Physician. When you use the "IN-NETWORK" system, you pay only a fee per visit. The rest of your covered medical expenses are paid in full.

The "OUT-OF-NETWORK" system allows you to use doctors and hospitals who are not participating in the network. Any care you receive that has not been directed by your

Primary Care Physician is considered to be "OUT-OF-NETWORK". When you use the "OUT-OF-NETWORK" system you will pay an annual deductible amount and the plan will pay 80% of allowable covered expenses. You have the choice of using either the "IN-NETWORK" OR "OUT-OF-NETWORK" system at any time.

A Prescription Drug Card with a co-payment charge at participating pharmacies is included in the UCPOS Plan.

CDPHP is a Health Maintenance Organization. You must select a CDPHP Primary Care Physician. If you have a family, each family member may have their own Primary Care Physician. General health care, referral to other doctors, and hospital admission must be directed by the Primary Care Physician. When your health care is directed by the Primary Care Physician, you pay only a fee per visit. Other covered expenses are paid in full.

A Prescription Drug Card with a co-payment charge at participating pharmacies is included in the CDPHP Plan.

Medical care obtained outside of the CDPHP HMO network is generally not covered except for emergency out of area care.

The MVP HEALTH PLAN (MVP HMO):

MVP

MVP is a Health Maintenance Organization. You must select a MVP Primary Care Physician. If you have a family, each family member may have their own Primary Care Physician. When your Primary Care Physician directs your health care, you pay only a fee per visit as most expenses are covered by insurance. A prescription Drug Card with a co-payment charge at participating pharmacies is included in the MVP Plan. There is also a mail-in prescription program. Medical care obtained outside of the MVP network is generally not covered except for emergency out-of-area care and for students attending college out-of-the-area.

Dental Insurance Plan

PLUS AND BASIC OPTIONS

You can select between two levels of Dental Insurance based on your dental care needs. You may also opt out of dental coverage. The following provides a summary of the dental options.

The Dental Plus Plan has the lowest annual deductible, full coverage for preventive dental care, high percentage payment of other expenses and includes orthodontic coverage for adults and dependent children. The Dental Basic Plan has lower premiums, a higher annual deductible, a lower percentage of payment for most dental services, and does not include orthodontic coverage.

You must complete a waiting period of a full enrollment year before you can transfer between dental insurance options. If you or your dependents enroll later than 31 days after your date-of-hire or qualifying event, benefit coverage for Major and Orthodontic Services will be reduced by 50% for 24 months.

Mail Order Prescription Program

 Members of Union's medical insurance plans can save themselves both time and money by utilizing the Mail Order Prescription Program. The Mail Order Program can be used for all long term prescriptions and works especially well for maintenance medications. Maintenance medications are those used to treat chronic conditions such as high blood pressure, asthma, diabetes or allergies. Employees using the UCPOS or CDPHP HMO Mail Order Program will receive a 90-day supply (three-month supply) of medication for just two co-payments. Those using the MVP HMO mail order drug plan, will receive a 3-month supply for 2.5 co-pays. The UCPOS and CDPHP HMO plans use CVS Caremark.com (1-888-292-6330 for their Mail Order Program or prescriptions may also be filled at any CVS Pharmacy. MVP uses Medco.com (1-800-716-3752). All Prescription Mail Order forms are available from Human Resources or your Health Insurance provider.
