Limited Disclosure/Non-Retaliation Acknowledgement
This form is used to protect the rights of both the Complainant and the Respondent, and includes a checklist of points that the college is asked to provide to make sure that you understand the conditions for moving forward. This includes all parties, witnesses, and advisers.
Notice of Appeal
This form is to be used by either party in the event of an appeal. Per the Sexual Misconduct Adjudication Process, both the complainant and the respondent are entitled to one appeal of an Adjudication Panel’s decision. An appeal must be filed within seven (7) days after the complainant’s and respondent’s meetings with the Administrative team handling the case.
To file a disciplinary complaint against a member of the Union College community under the Sexual Misconduct Adjudication Process, one must complete this Complaint Form. This document should contain basic information such as the time, date, location, and brief description of the allegation underlying the disciplinary complaint. This Complaint Form must contain sufficient detail to permit a reasonable person to understand the complaint being brought forward and to be able to adequately respond. The Respondent will be given access to the Complaint Form prior to filing a written response statement.

This statement should provide as much detail as possible about the facts surrounding the alleged conduct.
In response to the complaint brought under the Sexual Misconduct Adjudication Process, one must submit a written statement addressing the allegation in the Complaint Form. This statement should contain a full recollection of the alleged incident. This statement is the first opportunity to address the allegations made by the Complainant. The statement should communicate a recollection of events and context, as well as your reflections regarding the alleged events. This statement should provide as much detail as possible about the facts surrounding the alleged sexual misconduct.
