Health Requirements & Important Forms

The Wicker Wellness Center is eager to assist you with any health concerns you may have while at Union.  As per NYS law and Union College Policy, certain health requirements must be completed prior to the start of classes for all first-year or new incoming students. Returning students do not need to resubmit these requirements.  To ensure a smooth start to your college career, be sure to complete the following requirements by August 5th, 2016. The Fall 2016 Health Requirement Checklist will guide you through the process of completing your Union College Health Requirement.  Print a copy of the checklist and read all the directions carefully to be sure you have completed each step in its entirety.

Student Health Portal

You must register for your Student Health Portal using your Union College username (your Union College email account address minus the "").  Your email account will be set up as of May 1st (please note you will not be able to complete the Health Portal process prior to this date). If you have trouble registering, please contact our Information Technology Services Help Desk at 518-388-6400 or to reset your student health portal access.

Fall 2016 Student Health Insurance

Enrollment in student health insurance is automatic and will appear on your student account statement along with the Fall term bill in July. If you want to continue on your current health plan and do not wish to enroll in the student health insurance plan, you need to waive enrollment online at Arthur J. Gallagher & Co. by August 6, 2016.

Fall 2016 Insurance for International and Exchange Students

All international and exchange students must have health insurance through HTH Worldwide. International and exchange students will be automatically billed in September after arriving on campus. Payment can be made at the finance office in McKean House by cash, check, or wire transfer. If you have further questions, please contact the Office of International Advising.

Health Requirement Forms to be Completed by August 5th, 2016

*All forms below must be completed. Then, scan and upload the completed forms on the Student Health Portal (more information is above).

*Some students may need a PPD Test (Tuberculosis Test) and/or a chest x-ray (read the Tuberculosis Risk Form carefully to see what you need).  If needed, PPD tests and chest x-rays must be completed within the last 12 months prior to arrival on campus.
