As per NYS law and Union College Policy, certain health requirements must be completed prior to the start of classes. The Fall 2015 Health Requirement Checklist will guide you through the process of completing your Union College Health Requirement. Print a copy of the checklist and read all the directions carefully to be sure you have completed each step in its entirety.

*You must register for your Student Health Portal using your Union College email account. Your email account will be set up as of May 1st (please note you will not be able to complete the Health Portal process prior to this date). If you have trouble registering, call (518) 388-6120 to have us reset your student health portal access.
*Enrollment in student health insurance is automatic and will appear on your student account statement along with the Fall term bill in July. If you do not wish to enroll in the student health insurance plan, you need to waive enrollment online at Gallagher Koster by August 12, 2015.
Health Requirement Forms to be Completed by August 3rd
*Every incoming student must have had a physical exam within the last 12 months prior to arrival on campus.
*Some students may need a PPD Test (Tuberculosis Test) and/or a chest x-ray (read the Tuberculosis Risk Form carefully to see what you need). If needed, PPD tests and chest x-rays must be completed within the last 12 months prior to arrival on campus.
